This includes our books, online courses, downloadable resources, email communications, podcast, YouTube channel, and any in-person services scheduled through our website (collectively, the "Services").
By accessing or using any part of our website or Services, you ("you" or "User") agree to be bound by these Terms, our Privacy Policy, and our Medical Disclaimer, each of which is incorporated into these Terms by reference. These Terms form a binding legal agreement between you and Andalon Physical Therapy, Inc.

Revive PT & Pilates is a women's health physical therapy and Pilates practice located in Encinitas, California. We provide:

Books authored by Dr. Dawn D. Andalon — including The Pelvic & Core Reset™ — are protected by copyright. Purchase of a book grants you the right to read the book and apply its teachings to your personal practice. It does not grant you the right to reproduce, redistribute, teach, or commercially exploit the content of the book without express written permission.
Online programs and courses
Access to our online programs and courses is granted under the specific terms presented at the time of enrollment. Course content is licensed to you for personal use only. You may not record, redistribute, share login credentials, or otherwise make course content available to others.
7. User-Generated Content
If you submit reviews, comments, ratings, testimonials, photos, videos, or any other content to our website or Services ("User Content"), the following terms apply.
Your license to us
By submitting User Content, you grant us a worldwide, royalty-free, perpetual, irrevocable, non-exclusive, transferable, and sublicensable license to use, reproduce, modify, adapt, publish, translate, distribute, publicly display, and publicly perform your User Content in any media, for our marketing, promotional, educational, and business purposes. This includes the right to feature your testimonial or review on our website, in our advertising, in our books or courses, and on social media.
You retain ownership of your User Content. You may continue to use your User Content however you choose.

Testimonials
If you provide a testimonial about your experience with Revive PT & Pilates, you agree that:
Your testimonial reflects your honest, personal experience
We may edit your testimonial for length, clarity, or grammar without changing its substantive meaning
We may use your first name, last initial, age, city, and likeness (if you've provided a photo or video) in connection with the testimonial
You will not be compensated for your testimonial unless we have specifically agreed otherwise in writing
You may withdraw permission to use your testimonial at any time by emailing info@reviveptandpilates.com. We will remove your testimonial from our active marketing materials within a reasonable time after receiving your request, though we cannot remove it from materials already in distribution (such as printed books or already-delivered emails).
Our rights regarding User Content
We are not obligated to publish, display, or maintain any User Content. We reserve the right to remove, edit, or refuse to publish any User Content at our discretion, without notice, for any reason.
We do not pre-screen or actively monitor all User Content. If you believe any User Content violates these Terms or any law, please notify us at info@reviveptandpilates.com.

If you believe content on our website infringes your copyright, please send a written notice to info@reviveptandpilates.com with the following information, as required by the Digital Millennium Copyright Act:

8. Purchases, Payments, and Refunds
Books
Books authored by Dr. Dawn D. Andalon are sold primarily through Amazon and other third-party retailers. Refunds and returns for book purchases are governed by the policies of the retailer from which you purchased the book, not by Revive PT & Pilates directly.
Online programs and courses
Online programs and courses are offered through drdawnandalon.com and may be hosted on third-party learning platforms. Pricing, refund eligibility, cancellation rights, and refund timing for each program are described at the point of purchase. You agree to the program-specific terms presented at checkout in addition to these Terms.
Generally, refunds for online programs are available within a specified period after purchase (typically 14 days), provided you have not completed more than a defined percentage of the program. Specific refund terms control over this general statement.
Discovery Visits and in-person services
In-person services at our Encinitas studio — including Discovery Visits, physical therapy appointments, and Pilates sessions — are subject to the following policies:
Booking and payment: Appointments can be booked through our website or by contacting our office directly. Payment may be required at the time of booking or at the time of service, depending on the service type.
Cancellation policy: We require a minimum of 24 hours' notice for cancellation or rescheduling. Appointments cancelled with less than 24 hours' notice, or missed without notice ("no-shows"), may be subject to a cancellation fee equal to the full cost of the scheduled session.
Late arrivals: Appointments that begin late due to client delay will still end at the scheduled time. We cannot guarantee that the full session length will be available if you arrive late.
Refunds for in-person services: Pre-paid sessions are generally non-refundable but may be applied to future services. Specific refund decisions are made on a case-by-case basis. We offer a 100% Happiness Guarantee on every session, the specific terms of which are described at the time of booking.
Bonus codes and promotional offers
From time to time we offer bonus codes, discount codes, and promotional offers — including the $50 off Discovery Visit code provided with purchase of The Pelvic & Core Reset™ book. These offers are subject to the following terms:
One bonus code per person, unless otherwise specified
Bonus codes are non-transferable and have no cash value
Bonus codes cannot be combined with other offers unless explicitly stated
Bonus codes may have an expiration date as noted at the time of issuance
We reserve the right to modify or discontinue promotional offers at any time
Proof of purchase may be required to redeem a bonus code
Payment processing
Payments are processed through third-party payment processors. By making a purchase, you authorize us (or our payment processor) to charge the payment method you provide for the amount of your purchase, including any applicable taxes and fees. We do not store your full payment card information.
Pricing and availability
All prices are stated in US dollars unless otherwise noted. We reserve the right to change pricing at any time, but changes will not affect purchases already completed. We reserve the right to limit purchase quantities and to discontinue any product or service.
9. Health-Related Disclaimers
Not a substitute for medical care
The content on our website and within our Services — including books, online programs, videos, blog posts, and any other educational material — is for general informational and educational purposes only. It is not a substitute for individualized medical advice, diagnosis, or treatment from a qualified, licensed healthcare provider.

No provider-patient relationship from website use
Your use of our website, content, or Services does not create a physical therapist–patient, doctor–patient, or any other healthcare provider–patient relationship between you and Revive PT & Pilates or any of our staff. A provider-patient relationship is established only through a direct, in-person clinical evaluation by a licensed Revive clinician.
Individual results vary
Any testimonials, case examples, success stories, or client experiences shared on our website or in our content reflect individual experiences. Results vary based on many factors including individual physical condition, medical history, consistency of practice, and adherence to professional guidance. No specific outcome is guaranteed.
Emergencies
If you believe you are experiencing a medical emergency, call 911 or your local emergency services immediately. Do not rely on our website or Services in an emergency.

Loss of profits, business, business opportunities, savings, or goodwill
Loss of use or corruption of information or data
Loss of, or unauthorized access to, your information
Personal injury or property damage arising from use of our content, except where such liability cannot be excluded by law
Damages resulting from third-party goods, services, or content accessed through our website
Damages arising from your reliance on any information provided through our Services
The total cumulative liability of the Revive Parties for any claim arising from or related to these Terms, our website, or our Services will not exceed the greater of: (a) the amount you paid to us in the 12 months preceding the claim, or (b) one hundred dollars ($100).
These limitations apply whether the claim is based in contract, tort (including negligence), strict liability, or any other legal theory, and apply even if a Revive Party has been advised of the possibility of such damages.
Some jurisdictions do not allow the exclusion or limitation of certain damages. In those jurisdictions, our liability is limited to the maximum extent permitted by law.

You agree to indemnify, defend, and hold harmless the Revive Parties from and against any claims, damages, losses, liabilities, costs, and expenses (including reasonable attorneys' fees) arising from or related to:

Your violation of these Terms
Your violation of any rights of another person or entity
Your User Content
Your violation of any applicable law
We reserve the right to assume the exclusive defense and control of any matter subject to indemnification by you, in which case you will cooperate with us in asserting any available defenses.

16. Dispute Resolution
Informal resolution first
Before filing any legal action, you agree to first attempt to resolve any dispute informally by contacting us at info@reviveptandpilates.com with a written description of the dispute. We will attempt to resolve the dispute in good faith within thirty (30) days of receiving your notice.
Binding arbitration
If we are unable to resolve a dispute informally, any dispute, claim, or controversy arising out of or relating to these Terms or your use of our website or Services will be resolved through binding individual arbitration, rather than in court, except as otherwise provided below.
Arbitration will be conducted by JAMS or the American Arbitration Association (AAA), under their respective rules then in effect. The arbitration will take place in San Diego County, California, or, at your election, through a virtual proceeding. The arbitrator's decision will be final and binding.
Class action waiver
You and we agree that any dispute will be brought in an individual capacity, and not as part of any class, collective, or representative action. You waive your right to participate in a class action lawsuit or class-wide arbitration against Revive PT & Pilates.
Exceptions
The following claims are not subject to arbitration:
Claims for injunctive or equitable relief related to intellectual property
Small claims court actions, provided the claim qualifies under that court's jurisdiction
Claims that cannot be subject to arbitration under applicable law
Opt out of arbitration
You may opt out of the arbitration and class action waiver provisions by emailing info@reviveptandpilates.com within 30 days of first agreeing to these Terms, stating your name, contact information, and that you wish to opt out of the arbitration provisions. Opting out will not affect any other portion of these Terms.
17. Governing Law and Jurisdiction
These Terms and any dispute arising from or related to them are governed by and construed in accordance with the laws of the State of California, without regard to its conflict of laws principles.
Subject to the Dispute Resolution section above, any legal action that is not subject to arbitration must be brought exclusively in the state or federal courts located in San Diego County, California. You consent to the personal jurisdiction of those courts.
If you access our website from outside the United States, you do so on your own initiative and are responsible for compliance with local laws. We make no representation that our website or Services are appropriate or available for use in all locations.

We may update these Terms from time to time to reflect changes in our business, technology, legal requirements, or other factors. When we make changes, we will update the "Last Updated" date at the top of these Terms.

These Terms, together with our Privacy Policy and Medical Disclaimer, constitute the entire agreement between you and Revive PT & Pilates regarding your use of our website and Services. They supersede any prior agreements or communications regarding the same subject matter.
Severability
If any provision of these Terms is found to be invalid, illegal, or unenforceable by a court of competent jurisdiction, the remaining provisions will remain in full force and effect. The invalid provision will be modified to the minimum extent necessary to make it enforceable while preserving its original intent.
No waiver
Our failure to enforce any provision of these Terms is not a waiver of our right to do so later. No waiver of any provision will be effective unless in writing and signed by an authorized representative of Revive PT & Pilates.
Assignment
You may not assign or transfer your rights or obligations under these Terms without our prior written consent. We may assign these Terms at any time, including in connection with a merger, acquisition, or sale of assets.
Relationship of the parties
Nothing in these Terms creates a partnership, joint venture, employment, or agency relationship between you and Revive PT & Pilates.
