Back Pain Does Not Just Hurt. It Quietly Takes Over Your Entire Life.
It starts with the things you stop doing. The morning walk you skip because you’re not sure how your back will feel. The trip you don’t plan because you can’t predict a bad day. The activity you sit out because standing for two hours is not worth three days of paying for it.
You are not managing back pain. You are managing your entire life around it. That is what nobody names — and what every woman who walks through our door recognizes the moment we say it out loud.
At Revive, we do not help you get better at tolerating back pain. We find what is actually driving it — the spinal loading pattern, the pelvic misalignment, the core compensation your body has been running for years — treat that, and give you back the life you quietly stopped living.
If this sounds like you…
You wake up and the first thing you feel before your feet even hit the floor is pain — and you lie there for a moment dreading what the day is going to feel like
There is a constant dull ache in your lower back that never fully goes away — not when you sit, not when you stand, not even when you lie down
Sometimes the pain shoots down your leg without warning and stops you completely — mid-stride, mid-sentence, mid-life
You have a mental list of things you no longer do — not because you decided to stop but because your back decided for you
You have been told your imaging looks fine but you are still in pain
You lie awake repositioning yourself trying to find the one angle that does not hurt — and you cannot remember the last time you slept through the night without waking up in pain
You have tried so many things — and each one worked for a week, maybe two — and then the pain came back and you felt more defeated than before you started
We are here to tell you — your body is not failing you. The care you have received has been failing you. That changes here.
Most Women With Back Pain Have Never Actually Been Diagnosed. They’ve Only Been Treated.
Most women with persistent back pain have been through the standard sequence. Chiropractor. Massage. Stretching routines. Six rounds of physical therapy with a home exercise sheet at the end. Sometimes it helped temporarily. Then it came back. Because all of those treatments were aimed at the pain signal — not at what was generating it.
Back pain in women over 40 is frequently driven by a combination of factors that standard physical therapy never examines together: pelvic floor dysfunction, hormonal changes to connective tissue during perimenopause and menopause, and long-standing compensation patterns in the core and spine. Dr. Dawn’s Pelvic & Core Reset™ Method addresses all three as part of every evaluation.
When you identify what is actually causing the problem — not just where the pain is — the treatment becomes fundamentally different. And the results last.
So what does your back pain actually look like? Every woman’s experience is different.
Here is what we commonly see:
-

Chronic lower back pain or stiffness
-

Sharp or shooting pain into the glutes or legs
-

Sacroiliac or SI joint pain
-

Pain that is worse in the morning or after sitting
-

Back pain that flares during exercise or daily activities
-

Disc-related pain or sciatica
-

Core weakness or instability
-

Back pain that gets worse during perimenopause or menopause
Whatever your back pain looks like day to day — whether it’s a dull ache that never fully goes away or a sharp flare that stops you mid-movement — the experience is the same: you are constantly negotiating with your own body.
Back pain in women over 40 is often more complex than it appears. The pain you feel in your lumbar spine may be driven by pelvic floor dysfunction, hormonal changes to ligament laxity during perimenopause, or a compensation pattern your body developed years ago and has been running ever since. Finding out which one — and addressing it specifically — is the difference between temporary relief and lasting recovery.
You don’t need a referral to get started — California is a direct-access state.
WHAT OUR CLIENTS ARE SAYING.
WHAT OUR CLIENTS ARE SAYING.
Donna, 66 - Severe back pain & sciatica
“I came in barely able to move. The pain in my back and down my leg was constant. After working with the Revive team on my core strength, I cancelled my surgery consult. Now I'm running around with my grandson and helping care for my elderly parents. I never thought I'd feel this way again.”
PATRICIA, 54 — chronic lower back pain & SI joint dysfunction
"I had been waking up in pain every single morning for three years. I had tried everything — two different chiropractors, massage, physical therapy at another clinic. Nothing lasted more than a few days. After working with the Revive team I finally understood why my back kept hurting. Within six weeks, I was sleeping through the night and back to my morning walks. I wish I had found them years ago."
Susan, 61 — lower back pain & core weakness
"My doctor told me the MRI looked fine and that I just needed to strengthen my core. I had no idea where to start or what was safe to do. The Revive team assessed everything — my movement, my core, and things I never would have connected to my back pain. Three months later, I am back in the gym, hiking on weekends, and carrying my grandkids without bracing myself. This is the care I had been looking for."
Most insurance-based clinics cap your sessions, rotate your therapists, and discharge you on their timeline — not yours. At Revive, we operate outside that system entirely. That means longer one-on-one sessions delivered by a team that knows your history and is all trained in the same method — and a care plan built around what your body actually needs, not what an insurance company will approve. That's the difference between treating a problem and unlocking a potential.
When you pay directly for your care, every decision is made for one person: you.
At a traditional insurance-based clinic:
Sessions are typically 30 minutes and often rushed
You may see a different therapist each visit — with no shared approach to your care
Your discharge date is determined by your insurance — not your progress
Treatment follows a standard protocol designed for the average patient
The pelvic floor is rarely assessed, even when it is contributing to your pain
At Revive PT & Pilates
One-on-one sessions, never rushed, delivered by a team that's all trained in the same method
Your care plan is built entirely around your body, your history, and your goals
Treatment continues as long as you need it — no arbitrary end dates
Every provider is trained in Dr. Dawn's Pelvic & Core Reset™ Method
We integrate physical therapy and therapeutic Pilates for lasting results
We're happy to provide a superbill — a coded receipt with instructions — that you can submit to your PPO insurance for possible reimbursement.
Most women with back pain have been told what they have — but not why they have it or what is actually driving it. The type of back pain matters enormously because the treatment is completely different depending on the cause. Here is what that looks like in practice:
Lower back pain (acute and chronic)
Lumbar disc herniation or bulging disc
Lumbar spinal stenosis
Facet joint pain
Sacroiliac joint dysfunction
Back pain worsened by perimenopause or menopause
Muscle strain and chronic tension patterns
Post-surgical back pain and recovery
Back pain driven by pelvic floor dysfunction
Postural back pain from prolonged sitting or standing
Back pain related to osteoporosis or bone density loss
Back pain after hysterectomy or other pelvic surgery
Women who have tried other PT and not improved
Not sure whether your specific type of back pain is right for us? Your Discovery Visit is exactly where we figure that out together.
Every Type of Back Pain Has a Root Cause. This Is Where We Start.
A Clear Path Back to the Life You Stopped Living
01. Start With a Discovery Visit
Your first step is a conversation about where you actually are and where you want to be. If you're navigating pain, leaks, or a concern that needs expert care — that's a 60- minute Discovery Visit including a full evaluation with our PT team. If you're ready to start building strength and confidence with movement — that's a 60-minute Pilates Intro Session, one-on-one with a women's health specialist. Either way, you tell us what you have been experiencing, how long it has been going on, and what you want your life to look like. We listen. Then we tell you honestly whether we can help and exactly what that would look like.
02. We Find What Is Actually Driving Your Back Pain
A thorough one-on-one evaluation of your movement patterns, lumbar mechanics, hip function, pelvic floor involvement, core stability, and the full picture of what your body is actually capable of — because back pain almost never has a single cause. We find all of them.
03. We Build Your Program Around You
Your care plan is built entirely around what your body specifically needs. It may include manual therapy, therapeutic Pilates, shockwave therapy, movement re-education, or a combination. Nothing is generic. Nothing is guesswork. Every session builds on the last. Every decision is pointed at the same destination — the life you described in Step 01.
04. You Stop Being Someone With Back Pain
The goal is not symptom management. The goal is for you to exercise without fear, travel without planning around your condition, and move through your life with the strength and confidence you deserve. That’s what we are working toward. And it’s absolutely possible for you.
No referral needed. No pressure — just answers and a plan.

Questions Women Ask Before Starting Back Pain PT at Revive
-
Yes — and this connection is one of the most consistently overlooked causes of chronic back pain in women over 40. Your pelvic floor, deep core muscles, and lumbar spine are part of the same pressure management system in your body. When the pelvic floor isn’t functioning optimally — which often happens during perimenopause and menopause due to hormonal changes — it affects the stability of your entire lower back. Standard physical therapy rarely assesses this. We always do.
-
Physical therapy can produce real, lasting relief for most types of back pain — especially when the root cause is identified and treated rather than just the symptoms. Many of our clients come to us after years of managing pain with medication, chiropractic adjustments, or generic exercises, and achieve results they didn’t think were possible. That said, we’ll never promise permanent results without first understanding your specific situation. Your Discovery Visit is where we give you an honest answer about what’s achievable for your body.
-
It's not too late — and we've seen this enough times to say it with confidence. We regularly work with women who have had back pain for 5, 10, or even 20 years and who had been told there was nothing more that could be done. In the majority of cases, the right assessment reveals something that was missed — and the right treatment produces changes that feel nothing short of remarkable. The only way to know what's possible for you specifically is to come in and find out.
-
Yes — physical therapy is not only safe for women with osteoporosis or a history of back surgery, it's one of the most important things you can do for long-term recovery and bone health. The key is working with a specialist who understands how to load your spine safely, which movements to prioritize, and which to avoid for your specific situation.
At Revive, every provider is trained in Dr. Dawn Andalon's Pelvic & Core Reset™ Method — which includes specific protocols for women with osteoporosis, post-surgical recovery, and complex spinal histories. We assess your individual situation before building any program. Nothing is generic. Nothing is assumed.
If you have had back surgery or have been diagnosed with osteoporosis and are unsure whether PT is appropriate for you, your Discovery Visit is exactly where we answer that question honestly.
-
Chiropractic care and massage can provide temporary relief by addressing structural alignment and muscle tension. What they typically don't do is assess and treat the underlying movement patterns, core dysfunction, and pelvic floor issues that are often driving chronic back pain in women over 40.
At Revive, every provider is trained in Dr. Dawn Andalon's Pelvic & Core Reset™ Method — a proven approach developed over 22 years of women's health practice that addresses the full system, not just the point of pain. We look at how your pelvic floor, core, spine, and movement patterns are all connected and contributing to your back pain — something chiropractic and massage simply aren't designed to do.
And because we integrate therapeutic Pilates into your program, the strength and stability gains you make carry far beyond your sessions and into your daily life in a lasting way.
-
Most of our clients notice meaningful improvement within the first three to four sessions. A full treatment plan typically runs eight to twelve weeks, depending on the severity and duration of your back pain and your consistency with the home care component of your program. At your Discovery Visit, we'll give you a realistic timeline based on your specific situation — not a generic estimate.
-
There's no other practice in North County San Diego that combines women's health physical therapy with therapeutic Pilates in one integrated program — designed specifically for women over 40. Every provider on our team is trained in Dr. Dawn Andalon's Pelvic & Core Reset™ Method, which means every session is grounded in the same foundation — no matter which provider you see. Our sessions are one-on-one and unhurried, delivered by a team that knows your history and is all trained in the same method. And because we're a cash-pay practice, every decision we make is based entirely on what your body needs — not what an insurance company will approve.

No referral is needed. California is a direct-access state. If your care plan benefits from working with your doctor or specialist, we're always happy to collaborate with your existing care team — but you don't need to wait for anyone's permission to get started.

Revive PT & Pilates is a private-pay practice, which means we don't bill insurance directly.
We're considered out-of-network for PPO plans on our physical therapy services. However, we're happy to provide a superbill — a coded receipt with instructions — that you can submit to your PPO insurance for possible reimbursement. Many of our clients receive partial reimbursement this way.
Medicare does not typically reimburse for private-pay physical therapy, but many of our clients over 65 find the value of one-on-one specialist care well worth the investment. If you have questions about fees before booking, reach out and we'll walk you through everything — no pressure, no surprises.
Our cash-pay model is what allows us to provide longer, more personalized one-on-one sessions without the restrictions that come with insurance-based care. Every decision we make is based on what your body needs — not what an insurance company will approve.

The Women Who Come Here Stop Describing Themselves as Someone With Back Pain.
That’s the shift. Not less pain. Not better days. A different identity entirely — a woman who hikes, travels, picks up her grandchildren, and goes to bed without calculating tomorrow's risk.
The women who complete this program tell us they stopped being the person with the bad back. That’s what is available on the other side of a Discovery Visit — and it starts with one honest conversation about what’s going on in your body and what is actually possible.
