6. Health Information

Revive PT & Pilates is a physical therapy practice. As a healthcare provider, our handling of patient health information is governed by:

  • The Health Insurance Portability and Accountability Act (HIPAA) at the federal level

  • California's Confidentiality of Medical Information Act (CMIA) at the state level

Information you submit through our website — such as email opt-ins, contact form submissions, lead magnet sign-ups, and waitlist registrations — is generally not considered Protected Health Information (PHI) under HIPAA, because you are not yet our patient at the time of submission. This information is handled under this Privacy Policy.

Information you provide once you become a patient — including health history, treatment notes, billing records, and any health-related communications with our clinical staff — is PHI and is handled under our separate Notice of Privacy Practices, which is provided to all patients at intake.
