Revive PT & Pilates is a physical therapy practice. As a healthcare provider, our handling of patient health information is governed by:

California's Confidentiality of Medical Information Act (CMIA) at the state level
Information you submit through our website — such as email opt-ins, contact form submissions, lead magnet sign-ups, and waitlist registrations — is generally not considered Protected Health Information (PHI) under HIPAA, because you are not yet our patient at the time of submission.
